The Virginia Department of Health, Office of Health Equity (VDH-OHE) has created the Health Opportunity Index (HOI) as a solution to help communities identify specific factors resulting in poor health outcomes. By identifying these factors (determinants), communities can take actionable steps to improve health outcomes for all residents. The HOI works by combining various health factors and presents them in easy to understand maps and reports, allowing users to understand how different areas are impacted.
